Health and Emergency Care Forms:
All
pets must be accompanied by a certificate of good health signed by a
licensed veterinarian within 10 days of departure, as well as proof of
rabies vaccination. Also, the pet owner must sign a form authorizing emergency care for the pet if necessary. The driver will not begin transport without these signed forms. Pets younger than 8 weeks of age are not eligible for transport. What to Pack? We require the owner to supply 5 days of food for your pet's trip and ONE of your pet's favorite toys! EXPRESS PET TRANSPORT,L.L.C. will supply everything else - bottled water, ice, bedding, leashes and treats! ALL PETS MUST WEAR A COLLAR AND TAG DURING TRANSPORT!!! Only the items listed will be allowed during transport unless prior arrangements have been made with EXPRESS PET TRANSPORT,L.L.C.. Rescheduling and Cancellations: To best service our customers and their babies, EXPRESS PET TRANSPORT, L.L.C. works hard to optimize our delivery schedules. We ask that you provide as much notice as possible of any rescheduling or cancellation. (see transport contract) Departure and Arrival: EXPRESS PET TRANSPORT, L.L.C. must be able to contact responsible parties at the scheduled departure and arrival times and places. For this reason, we require 2 contact names, telephone numbers and addresses for both the sender and the receiver of the pet. (see transport contract) Reservation Form
